The Chesterfield Health and Wellbeing Partnership Board exists to provide a platform to support local partners in working together to improve health and wellbeing, and reduce health inequalities in Chesterfield.
The partnership exists:
- to oversee the development and implementation of an agreed Chesterfield Health and Wellbeing Partnership Board Plan to respond to existing and future health and wellbeing issues in Chesterfield
- to ensure that the Chesterfield Health and Wellbeing Partnership Board plan aligns with the Derbyshire Health and Wellbeing Strategy, and Chesterfield Health and Wellbeing Partnership Board priorities
- to monitor and review the delivery and performance of projects and interventions contained in the Chesterfield Health and Wellbeing Partnership Board plan
- to identify, share and help secure resources to support the Chesterfield Health and Wellbeing Partnership Board work related to improving health and wellbeing and reducing health inequalities
- to oversee responsibilities and funding devolved to sub-groups (networks)
- to oversee a commissioning panel to make decisions on grant and funding applications
The priorities of the partnership are to:
Tackle the four main risk factors that lead to poor health:
- smoking
- inactivity
- poor diet
- alcohol consumption
Support good mental health:
- suicide prevention
- children and young people mental health
Support communities to be resilient and independent:
- housing
- falls prevention
- cost of living
- winter preparedness
Tackle child poverty and enable children and young people in Derbyshire to start well:
- school readiness
- financial inclusion
